Description
Trade name drug: HartMedin ® tablets (HeartMedin ® tablets).
International non-proprietary name: Pimobendan.

- used in dogs with congestive heart failure due to AV valve insufficiency or dilated cardiomyopathy.

Dosage form: tablet.
Tablets weighing 992 mg contain 10 mg of the active ingredient pimobendan (10.4 mg micronized pimobendan in silicon oxide), and auxiliary components: anhydrous citric acid – 480 mg, F-Melt type M – 473.6 mg, silicon oxide – 16 mg and magnesium stearate – 12 mg.
In appearance, HartMedin ® tablets are white, oval, uncoated tablets with a notch in the middle.
The medicinal product is packaged in appropriate blisters. Blisters are packed in individual cardboard boxes:
Tablets 10 mg are packaged in 10 tablets in a blister, which are packed in individual cardboard boxes of 3 (30 tablets).
HartMedin ® tablets are stored at temperatures from 0 ° C to 25 ° C.
Shelf life – 3 years from the date of production.
II. Pharmacological properties
Pharmacotherapeutic group: cardiotonic agent.
Pimobendan, which is part of HartMedin ® tablets, has a positive inotropic effect and a pronounced vasodilating effect. In contrast to cardiac glycosides, the positive inotropic effect of the drug is due to two mechanisms: an increase in the sensitivity of cardiac muscle fibers to calcium and inhibition of phosphodiesterase III. The vasodating effect is associated with inhibition of the activity of phosphodiesterase III. Due to the presence of positive inotroin and vasodilating action in heart failure, the drug increases the force of heart contractions and reduces both preload and afterload.
After oral administration, pimobendan is rapidly absorbed into the bloodstream, and its bioavailability is 60-63%. The plasma half-life of pimobendan is 0.4 ± 0.1 hours, and the half-life of its active metabolite is 2.0 ± 0.3 hours. Most of the metabolites of pimobendan are excreted in the faeces of animals.
HartMedin ® tablets, according to the degree of impact on the body, are classified as low-hazard substances (hazard class 4 according to GOST 12.1.007-76), do not possess sensitizing, embryotoxic, teratogenic and mutagenic effects.
III. Application procedure
HartMedin ® tablets administered orally for the treatment of cardiovascular disease in dogs. It is forbidden to use HartMedin ® tablets in dogs with severe liver damage, since the drug is mainly metabolized in the liver.
The combined use of HartMedin ® tablets with calcium channel blockers and the beta-blocker irropranolol is prohibited, since these substances weaken the effect of pimobendan.
The dosage can range from 0.2 mg to 0.6 mg pimobendan per 1 kg of body weight per day on the recommendation of a veterinarian, as well as depending on the severity of the disease and the condition of the animal. The daily dose is divided into two doses (see table):

In the case of congestive heart failure, HartMedin ® tablets are used for life in animals with the selection of an individual dose.
HartMedin ® tablets are given to the animal one hour before feeding.
In case of an overdose of the drug, the animal may experience vomiting, apathy, tachycardia, heart murmurs and a decrease in pressure. In case of overdose, you should stop giving the drug and contact your veterinarian. In this case, the immediate use of adsorbents (for example, activated carbon) is recommended.
The peculiarities of the action of the drug during its first use and withdrawal were not revealed.
Violations of the scheme of using HartMedin ® tablets should be avoided , as this can lead to a decrease in effectiveness. In case of skipping the next dose of the drug, it is carried out in the same dose according to the same scheme.
When using the drug HartMedin ® tablets in accordance with this instruction, side effects and complications, as a rule, are not observed.
With increased individual sensitivity of the animal to the components of the drug and the development of allergic reactions, the use of the drug is discontinued and desensitizing and symptomatic therapy is carried out.
When using HartMedin ® tablets with calcium channel blockers and the beta-blocker ipranolol, the effect of pimobendan will be reduced.
